Simplifying client referrals for professionals
At Wessex Cancer Support, we understand the importance of providing holistic, personalised support for our clients. Our Professional Referral Form is designed to make the referral process seamless, ensuring clients/patients receive the support they need.
Please note, our services only available to people 18 years of age and older.
Why refer your patients or clients to us?
We are proud to offer non-clinical support services that will be tailored to fit every individual’s unique cancer journey.  When we receive your referral, we will contact your client within seven days to have a welcome chat and let them know what the next steps will be. Should your patient/client choose to proceed, we will arrange for one of our Wellbeing Coordinators to have a one-to-one meeting with them, either face to face or remotely.  During this meeting, the patient/client will be given the time to identify what is important to them.
The Wellbeing Coordinator will collect any need-to-know information and collaboratively devise their personalised support plan.
If the needs of the patient/client go beyond the services we offer then the patient/client will be sensitively signposted on to more appropriate services.
We really value the opportunity to collaborate with referring professionals and are always open to thoughtful conversations about any concerns you’d like to highlight for your patient. Please note, our services only available to people 18 years of age and older and who live or work within the Wessex region. By referring your client to WCS, you initiate contact with our service. From there, we will assess their needs and determine which of our support services is most appropriate for them.

How to refer a patient or client to us
1. Fill out the referral form
Provide us with the necessary details about your client.
2. Submit the form
Once completed, submit the form through our secure online portal.
3. Confirmation
You will receive a confirmation email acknowledging receipt of your referral.
4. Follow-up
Our team will contact your client within seven days.
