Simplifying client referrals for professionals
At Wessex Cancer Support, we understand the importance of providing holistic, personalised support for our clients. Our Professional Referral Form is designed to make the referral process seamless, ensuring clients/patients receive the support they need.
Why refer your patients or clients to us?
- Personalised approach: When a referral is received, our Wellbeing co-ordinators will arrange a time to have a 1-1 chat with the person and will design a personalised support plan with them, based on what they have identified is important to them.
- Seamless process: Our referral process is straightforward and efficient, allowing you to refer clients with ease and ensuring confidentiality is maintained.
- Collaboration: We value the partnership with referring professionals and are open to discussing any area of concern you have for your patient/client that you may wish to highlight.
How to refer a client
- Fill out the referral form: Provide us with the necessary details about your client and the services you think they require.
- Submit the form: Once completed, submit the form through our secure online portal.
- Confirmation: You will receive a confirmation email acknowledging receipt of your referral.
- Follow-up: Our team will contact your client within 7 days to have a welcome chat and let them know what the next steps will be.
